As of April 1, 2019, GSFA has discontinued its accidental death and dismemberment insurance coverage with American Income Life. Please make sure your department is aware of this important change. Firefighters should no longer be contacted by American Income Life representatives.

As of August 1, 2019, GSFA has discontinued its life insurance coverage with Cotton State Insurance. The Board has chosen to offer a higher level of accidental death and dismemberment insurance rather than to continue to offer this small life insurance benefit, which came with a high premium.   

About Provident
Since 1928, Provident has offered accident and health insurance products to help protect the needs of emergency responders and their families and provides benefits for nearly 9,000 fire departments nationwide. Most importantly, Provident does not have representatives who will be contacting GSFA members directly. As long as a firefighter’s GSFA membership dues are current at the time of the incident, he or she is covered by this group policy.

Claims Process
Provident will handle all claims directly. When a claim needs to be filed, please use the following process:
  • The GSFA member, fire department representative or beneficiary should contact GSFA Business Manager Beth Brown at 770-914-7774 or bbrown@gsffa.org to report the incident.
  • GSFA will confirm that the individual is an active member, active retired member or retired member who is current on his or her dues at the time of the incident. 
  • GSFA will provide a Provident claim form to the individual, fire department representative or beneficiary for completion. GSFA must complete the Employer’s Statement portion of the claim form. Beneficiary Designation
At the time that membership dues are paid, GSFA members should establish and/or review their beneficiary designation. GSFA will maintain a physical or electronic beneficiary form for each member in a secured location and present it at the time of claim for those that choose to submit a Provident beneficiary form. In the event that GSFA has a physical form on file for the member dated April 1, 2019 or later, the physical form submitted by the individual will be presented at the time of claim.  
  
If the member does not have a beneficiary designated in the GSFA database nor has completed a Provident beneficiary form, or if all named beneficiaries die with or before the member, the death benefits may be payable in the order listed below: 
  1. Spouse;
  2. child or children;
  3. parents;
  4. siblings; or
  5. estate of the insured person.
If a GSFA member would like to name a specific beneficiary or beneficiaries) other than relying on the policy provisions outlined above, then he or she must complete this form: https://www.providentins.com/wp-content/uploads/2018/09/10_Beneficiary-Designation.pdf and return it to bbrown@gsffa.org or mail it to GSFA, 191 Peachtree Street, NE, Suite 700, Atlanta, GA 30303.
